Notification of incidents related to the medicine chain in a teaching hospital

Objective: To analyze the incidence and notifications of events related to the drug chain in a teaching hospital. Methods: Quantitative study, carried out in a teaching hospital in Salvador, Bahia, Brazil. Data were collected from notifications made between January / 2016 to December / 2018. Descriptive analysis of the data was performed. Results: The incidence of incidents related to the drug chain varied between 0.2 and 2.27 per 100 hospitalizations, with a higher proportion in adult individuals (49.36%), female (54.94%), black race (80.26%), little education (56.66%) and coming from Salvador (61.80%). The most reported incidents were administration errors (52.36%), prescription errors (22.32%) and dispensing errors (16.31%), with possible problems with scheduling problems (19.31%) and lack of attention from professionals (16.72%). Most incidents occurred during the day (51.93%), being notified by the pharmacist (69.53%). Among the notifications, there was 13.0% of damage to patients. Conclusion: There was an increasing variation in the reported incidents related to the drug chain in the years of the study and about 13.0% of these were classified as adverse events. The findings give visibility to the event profile and are essential for preventive actions.
