Objective: To evaluate the effects of virtual reality (VR) on gait functionality and perception of change in individuals with chronic hemiparesis. Methods: Experimental, longitudinal, prospective, single-arm pilot clinical study. The sample consisted of hemiparetic individuals undergoing intervention in a virtual reality-based therapy. In the initial evaluation (AV1), the Timed Up and Go (TUG) was used to analyze mobility and after 12 VR sessions, in the final evaluation (AV2), the Perceived Change Scale (PCS) was added. To verify the normality of the data, the Shapiro-Wilk test, the T-Student or Wilcoxon test were used to compare the data (p ≤ 0.05) and the Effect Size (ES) by the Cohen formula (d) for the size of the effect. Results: Ten hemiparetic individuals (64.6 ± 9.53 years) performed the TUG (AV1) in 14.59 ± 5.03 seconds and AV2 in 13.96 ± 4.64 seconds (p = 0.18) and the EF had a negligible effect (d = 0.14). The Free Step game showed a significant difference between the first and last session (p = 0.004) and in PCS the values obtained were 2.57 ± 0.3 of three points. Conclusion: VR did not promote significant improvement in functional mobility, but individuals reported positive changes in some components of the PCS.
